Case Planner - Prevention Program for Families
Cardinal McCloskey Community Services
A community service organization in New York is seeking a Case Planner to support at-risk children and families facing serious challenges. You will guide families through a structured treatment model while conducting assessments and developing tailored service plans. This role offers an opportunity to build meaningful relationships and help families achieve lasting change.
