Memory Care Coordinator

Terrabella Senior Living employs a Memory Care Coordinator at the Gardens of Gainesville community. The role focuses on program development, implementation, and resident support in an environment designed to protect dignity and safety for residents with dementia. Responsibilities Design, schedule, and facilitate Memory Care programs incorporating life skills and dementia‑appropriate activities. Partner with the Memory Care Director to ensure a variety of appropriate activities are available throughout the day and evening, and that Care Managers are actively involved and engaged with activities. Collaborate with the Memory Care Director in compiling, coordinating, and executing a comprehensive and varied program of activities to meet the broad spectrum of interests and capabilities of Memory Care residents. Provide ongoing in‑service education to Memory Care staff relative to the disease processes and population being served. Prior to move‑in, review the resident’s preferences and needs, educating Care Managers of the same. Facilitate the Service Plan and Daily Assignment Sheet development to ensure excellence in service delivery, safety, hydration, and resident engagement in programming. Act as the community champion in Memory Care programming and activities. Partner with dining services to ensure meeting individualized resident needs and preferences; participate in dining experience. Understand resident changes in condition and report to the nurse. Establish a cooperative relationship with the local Alzheimer’s Association chapter or other Alzheimer’s advisory organization. Develop specific programming and approach mechanisms that are tailored to meet the needs of residents with a diagnosis of Alzheimer’s Disease or related Dementia. Create a therapeutic environment focusing on first impressions and resident engagement. Quality Assurance and Regulatory Compliance Strive for excellent quality care and service delivery. Develop and implement appropriate plans of action to correct identified deficiencies and other regulatory compliance. Develop a thorough working knowledge of current and evolving state laws, regulations, policies, and procedures dictated for residents, and ensure compliance. Qualifications One (1) year of dementia care experience. One (1) year in a leadership role.
