Job Description

The LGH Hospital at Home program is launching on January 11th, seeking Registered Nurses to provide in-home patient care and assist with patient enrollment in the LGH hospital. Nurses will be primarily responsible for in-home visits and will work closely with the Hospital at Home team. Effective orientation in December will prepare the team for success in this innovative program.

Responsibilities

• Conduct patient assessments and provide nursing care during in-home visits.
• Assist with patient enrollment processes for the LGH hospital program.
• Maintain communication with hospital staff regarding patient care and progress.
• Participate in team meetings and training sessions as needed.

Required Experience

• Active Registered Nurse licensure.
• Experience in home health or community nursing preferred.

Certifications/Licensure

• Current CPR certification.
• State RN license required.
