Nurse Practitioner

As a nurse practitioner or physician assistant at Conviva, you will work within a collaborative, multidisciplinary team focused on seniors. Role & Responsibilities You will provide comprehensive care to seniors following clinical standards, collaborate daily with your care team to ensure coordinated, high‑quality care, manage referrals and care coordination (hospital/SNF, DME, home health), participate in daily huddles, document care with support from quality‑based coders, support on‑call rotation, and ensure compliance with licensing and accreditation requirements. Required Qualifications Master’s Degree as a nurse practitioner or completion of a physician assistant program with board certification. Active, unrestricted NP or PA license in the state of practice. Two years of experience in value‑based care, geriatric or primary care, managing adults aged 65+ with chronic conditions. Computer proficiency (EMR, email, etc.). Compliance with Humana/Senior Bridge's Tuberculosis (TB) screening program – you must be screened for TB if selected. Preferred Qualifications Experience in Medicare Advantage settings. Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain them. Familiarity with HEDIS quality indicators and Medicare guidelines. Additional Information Scheduled Weekly Hours: 40 Pay Range: $123,800 - $170,300 per year Job is eligible for a bonus incentive plan based on company and/or individual performance.
