Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Collaborating closely with a multi-disciplinary healthcare team to develop comprehensive treatment and discharge plans based on thorough psychosocial assessments of patients.

• Representing Redesign Counseling Hub at community resource group meetings, building partnerships that enhance the support network available to our patients.

• Providing counseling and crisis intervention services to patients experiencing emotional and behavioral challenges, utilizing a range of therapeutic techniques and approaches.

• Communicating effectively with physicians and other healthcare professionals to discuss patient progress, convey needs and concerns, and address any immediate issues.

• Maintaining accurate and timely clinical documentation, including assessment notes and progress reports in accordance with departmental policies and legal requirements.

Patient Population / Client Focus\n

This role focuses on a diverse patient population in a detox center, including those dealing with substance use disorders and co-occurring mental health issues. Your expertise will directly influence their recovery journey by providing tailored support that addresses both their psychological and social needs.

Qualifications\n

To be considered for this position, candidates must hold a Master’s degree in Social Work (MSW) and possess a valid LCSW license. Additionally, candidates should have:
