Patient Care Coordinator, Registry, Medical Oncology - Hyde Park

Job Description Join a world‑class academic healthcare system, UChicago Medicine, as a Patient Care Coordinator at our University of Chicago Medical Center in Hyde Park. In this role, you will work closely with our Medical Oncology teams, supporting exceptional patient care and coordination. This position is a 100% onsite opportunity, and candidates must be based in the greater Chicagoland area. Under direct supervision the patient care coordinator will be responsible for guiding patients through the health care system and ensuring those patients and their caregivers can access culturally and linguistically appropriate services in a timely and cost-effective manner. This position is responsible for the complex coordination of care for high acuity patents across multiple departments, ancillary areas, and UCM Network sites. Hybrid position that requires on‑site support for Hyde Park main campus and UCM Network sites. Essential Job Functions After initial new oncology appointment is scheduled serves as a single point of contact with new Oncology patients and is responsible to schedule appointments and coordinate referrals for testing, imaging, procedures, social work, physical therapy, occupational therapy, chaplain or spiritual services, geriatric-oncology, psycho-oncology, nutrition, palliative care, pharmacy clinic, SOCARE clinic, labs, clinical trials, and other care and support services at the hospital or in the community; contacts patients via phone, email, and/or letter to schedule, confirm, and send out reminders regarding appointments. Provides general education to patients and their caregivers about the care plan; Follows up with patients at regular intervals as defined to ensure that patients arrive to scheduled appointments on time and prepared. Fields patient and caregiver calls and questions related to appointments, ancillary services, or other matters related to coordination of care. Obtains prior authorizations, medical records, slides, specimens, faxes, and other items necessary for coordination of care; follows up with outside labs and other ancillary services for testing results and medical records and shares results with appropriate services as needed, including submitting results to Medical Records and patient’s community physician(s). Initiates and prepares FMLA/Disability paperwork with patients, completes appropriate forms as directed by Nurse Navigator (for example, screening assessments); Documents linkage to care services in the electronic medical record, including information provided to patients and their caregivers about appointments and referrals; Creates EPIC notes for supporting documentation Provides patient navigation and care coordination across UCM Network Sites Performs other related duties as assigned. Initiates and prepares FMLA/Disability paperwork with patients, completes appropriate forms as directed by Nurse Navigator (for example, screening assessments) Documents linkage to care services in the electronic medical record, including information provided to patients and their caregivers about appointments and referrals Creates EPIC notes for supporting documentation Build relationships with hospital staff, clinicians and physicians involved in the care of the oncology patients and with members of the community with whom UCM has strategic partnerships with for referral services. Performs other related duties as assigned. Required Qualifications High School diploma required. Bachelors preferred Minimum of two years’ experience working in a high-volume complex healthcare environment Excellent telephone and personal communication skills are required as is proficiency with computer systems and programs Ability to multi-task and prioritize is crucial to success in this position Compassionate listening skills are required for patient and family distress related to new diagnosis Ideal candidate has strong interest and desire for extensive patient and family interaction Requires experience with and high commitment to customer service Uses discretion and good judgment, and maintains patient confidentiality Must be able to independently and professionally interact with clinicians and personnel, internal and external to UCM Position Details Job Type/FTE: Registry/PRN (0.10 FTE) Shift: Days (As needed): Availability between 7:30 am - 5:00 pm Work Location: Hyde Park Unit/Department: Medical Oncology CBA Code: Non-Union In this role, we anticipate that you will generally work approximately 8 hours per week as needed. We anticipate that you will generally be scheduled to work the Dayshift, from approximately 7:30 am - 5:00 pm and will not be scheduled for on-call shifts. We anticipate you will be scheduled to work 75% at the Hyde Park campus and 25% Remotely. The days of the week that you are scheduled will be Monday-Friday.
