Role Description The Care Navigator is a full-time, hybrid role based in Mount Vernon, OH, with the flexibility to complete some responsibilities from home. This role supports patients and families by guiding them through the healthcare system, helping them understand care plans, appointments, and available resources. Day-to-day responsibilities include coordinating referrals, scheduling follow-up visits, and maintaining regular contact with patients to monitor progress and address barriers to care. The Care Navigator will collaborate with physicians, nurses, social workers, and other care team members to ensure smooth transitions of care and timely communication. Additional tasks include documenting patient interactions, tracking outcomes, educating patients on hospital and community resources, and supporting initiatives that improve access, engagement, and overall patient experience. Qualifications Strong patient advocacy and customer service skills, with the ability to build trust, listen actively, and communicate clearly with patients, families, and care teams. Organizational and coordination skills to manage multiple patients, track appointments and referrals, and follow through on care plans in a timely manner. Proficiency with electronic health records (EHR), basic computer applications (e.g., email, spreadsheets), and documentation standards. Knowledge of healthcare systems, care coordination, and community resources; prior experience in a hospital, clinic, or social services setting is preferred. Ability to work effectively in a hybrid environment, managing time independently while collaborating closely with on-site and remote team members. Excellent verbal and written communication skills, including the ability to explain medical information in clear, understandable language. High school diploma or equivalent required; an associate’s or bachelor’s degree in a health-related field, social work, or a similar discipline is preferred.
