Join a growing clinical team dedicated to providing high-quality, patient-centered care within post-acute and long-term care settings. We are seeking a motivated  Nurse Practitioner or Physician Assistant to manage the ongoing medical needs of residents in the Sarasota  area.

Key Responsibilities

• Patient Management: Conduct comprehensive evaluations and follow-up visits for residents in skilled nursing facilities (SNFs).
\n
• Clinical Strategy: Develop, implement, and adjust individualized treatment plans based on patient needs.
\n
• Collaboration: Work closely with physicians, nursing staff, therapists, and facility leadership to ensure cohesive care.
\n
• Documentation: Maintain accurate and timely patient records using Electronic Health Record (EHR) systems.
\n
• Care Coordination: Participate in interdisciplinary care planning meetings to optimize patient outcomes.
\n

Qualifications & Requirements

\n
• Licensure: Valid license as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Florida.
\n
• Skills: Strong clinical assessment skills and a professional demeanor.
\n
• Communication: Excellent interpersonal skills to effectively collaborate with a diverse care team and interact with patients/families.
\n
• Logistics: * Ability to commute daily to  Sarasota, FL (Required).
\n
• Ability to relocate to the area prior to starting work (Required).
\n
• Traits: Attention to detail and the ability to work independently while remaining a team player.
\n
