This role will provide direct healthcare services to patients under the direction and responsibility of our team of Physicians.Supervisory Responsibilities: NoneEssential Duties/Responsibilities(if Nurse Practitioner)Assess and interpret medical and psycho-social histories; perform physical examinations (including gynecological and breast examination as appropriate; identify problems; formulate a plan of treatment; provide education and referral (as indicated); and evaluate the effectiveness of the interventions prescribedIndependently assess and treat patients as clinically indicated. Consult with or refer clients with abnormal conditions or other medical, psychological, or social problems to the collaborating physician or another appropriate referral course when indicatedPerform or order laboratory tests and interpret the results as part of health management. Report communicable diseases in a timely manner to the appropriate Health Department as per health center protocolOrder medicationsFollow established treatment guidelines as indicated by national guidelines or Health Center protocolsMeet clinical quality performance measures (UDS and HEDIS measures)Conduct health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment optionsComplete the client's clinical record documentation in a timely manner specific to the applicable policies and procedures of the health centerParticipate in quality improvement/assurance activitiesParticipate in continuing medical education activities to enhance professionalism and to ensure quality of careProvide on-call coverage as scheduledWork collaboratively with all members of the health center to build a supportive teamApply infection control measures according to Health Center Policies and ProceduresMaintain professionalism and adhere to client confidentialityPerform other duties as assigned(if Physician Assistant)Triage new and existing patients to determine acuity of immediate complaint;
