Job Title: Licensed Masters Of Social Worker Profession: Social Services Specialty: Case Management Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 1 year required License: Washington license required Certifications: BLS, WA License Must-Have: Acute Care Experience, Discharge Planning/Care Coordination Experience Description: Must have a Washington license. Start date is as soon as possible. Shift is days, working 5 days a week. Every other weekend is required. Candidate must have experience in acute care and discharge planning. Strong preference for candidates with diverse case management experience, including Med Surg. The average case load is 18-24 patients, with weekends having up to 30 patients. Candidates will work in all areas of the hospital. Experience with the charting system EPIC is required. This position requires the ability to float between different units as needed. Candidates must commit to working 3 of the 5 major holidays. This role focuses on providing assessments, care coordination, and documentation of care delivery. Candidates must demonstrate knowledge of cultural issues and effective communication strategies for diverse patient populations.
