Job Summary:

Position Title: LMSW
Location: Everett, WA 98201
Duration: 8- 12 week
Work Schedule: Day 5x8-Hour (07:30 - 16:00)

Responsibilities for this position include, but are not limited to:

• Conduct biopsychosocial assessments for geriatric patients
• Develop, implement, and update individualized care plans
• Provide 1:1 patient care and facilitate group counseling sessions
• Document assessments, interventions, and progress accurately in EPIC
• Monitor patient conditions and report changes to the care team
• Collaborate with interdisciplinary teams to coordinate patient care
• Support patients with complex medical, mental health, and substance-use needs
• Ensure compliance with clinical protocols and safety standards

Will this traveler need to float between like units? Yes will work in all areas of the hospital including Colby and Pacific Campus' (less than 2 miles from each other

Qualifications:

• Must have Acute Care Experience, Discharge Planning/Care Coordination Experience
• Strong preference for a candidate with diverse CM experience - Med Surg, LLOS, ED
