Position Overview

The Community Support Facilitator (CSF) provides leadership, coordination, and oversight of services for individuals receiving Independent Living Services (ILS) and Supported Living Services (SLS). Working closely with the Program Director, the CSF supervises and supports direct service staff, coordinates individualized services, and helps ensure participants receive consistent, person-centered support.

The CSF serves as an important connection between participants, families, staff, Regional Centers, service providers, and community partners while ensuring services promote independence, health, safety, dignity, and successful achievement of individual goals.

What You'll Do

Participant Support & Advocacy

• Provide guidance and support to individuals with developmental and intellectual disabilities, including individuals with behavioral, medical, psychiatric, or other complex support needs.
• Support individuals in developing and maintaining independent living skills.
• Promote community integration, social connections, self-advocacy, and meaningful participation in the community.
• Ensure services are delivered according to each individual's Individual Program Plan (IPP), Individual Service Plan (ISP), and identified goals.
• Advocate for the rights, preferences, choices, and well-being of individuals served.
• Support individuals in accessing community resources, benefits, employment opportunities, transportation, and other services.

Staff Supervision & Leadership

• Supervise, guide, and mentor Community Living Instructors and House Managers.
• Provide staff with direction regarding participant-specific needs, routines, behavioral strategies, and service expectations.
• Assist with onboarding and training new employees assigned to participant homes.
• Monitor staff performance and provide constructive feedback and coaching.
• Address performance concerns and communicate significant issues to the Program Director.
• Ensure adequate staff coverage and appropriate staffing levels are maintained.
• Promote teamwork, accountability, professionalism, and consistent service delivery.

Service Coordination & Individual Planning

• Coordinate individualized services in collaboration with participants, families, staff, Regional Centers, and other service providers.
• Participate in IPP/ISP meetings, team meetings, case conferences, and service planning meetings.
• Monitor implementation of participant goals and identify areas requiring additional support.
• Ensure behavior support plans, health plans, and other individualized support plans are implemented appropriately.
• Coordinate appointments, services, and resources as needed to support participant outcomes.
• Maintain communication with families, conservators, Regional Center representatives, and other professionals involved in the individual's services.

Behavioral & Crisis Support

• Assist staff in implementing behavior support strategies and individualized behavior intervention plans.
• Provide guidance regarding de-escalation and crisis prevention strategies.
• Respond to emergencies and provide on-site support when necessary.
• Assist staff in addressing challenging or high-risk situations while maintaining participant dignity and safety.
• Ensure incidents and suspected abuse are reported according to agency and regulatory requirements.
• Review incident reports and communicate significant concerns to the Program Director.

Documentation & Compliance

• Maintain accurate, timely, and complete participant documentation.
• Monitor progress notes, service records, incident reports, and other required documentation.
• Ensure services are delivered in accordance with A Bright Future policies and applicable DDS, Regional Center, Title 17, and other regulatory requirements.
• Maintain participant files and ensure documentation is audit-ready.
• Participate in quality assurance reviews and assist with corrective action when needed.
• Ensure staff maintain required certifications, training, and documentation.

Quality Assurance & Program Operations

• Monitor the quality and consistency of services provided at assigned program sites.
• Conduct routine home and program visits to assess service quality and participant satisfaction.
• Identify service gaps and recommend improvements.
• Ensure homes and program environments are safe, organized, and appropriate for individuals served.
• Collaborate with the Program Director to address operational and staffing concerns.
• Assist with Regional Center audits, IHSS reviews, and other program monitoring activities.
• Promote continuous quality improvement throughout the program.

Qualifications

Education

• Bachelor's degree in Psychology, Human Services, Social Work, Special Education, or a related field preferred.

OR

• Four (4) or more years of relevant experience working in the developmental disabilities or human services field may be considered in lieu of a degree.

Experience

• Minimum of two (2) years of experience working with individuals with developmental or intellectual disabilities.
• Previous supervisory, lead, or service coordination experience preferred.
• Experience supporting individuals with behavioral, medical, psychiatric, or other complex needs preferred.
• Experience working with California Regional Centers and/or ILS/SLS programs preferred.

Minimum Requirements

• Must be at least 18 years of age .
• Valid California driver's license, vehicle registration, and automobile insurance.
• Reliable transportation and an insurable driving record.
• Willingness and ability to travel between participant homes, program locations, and community settings .
• Successful completion of required DOJ/fingerprint clearance.
• Successful completion of required drug screening and TB clearance.
• CPR and First Aid certification or willingness to obtain certification as required by the agency.
• Ability to work a flexible schedule based on participant and program needs.

Physical Requirements

This position may require the ability to:

• Sit, stand, walk, bend, kneel, and climb stairs.
• Travel between participant homes and community locations.
• Assist individuals with mobility needs, including wheelchair transfers.
• Respond appropriately during medical or behavioral emergencies.
• Implement CPR, First Aid, and Pro-ACT procedures when required.
• Push, pull, or lift up to 60 pounds .
• Provide physical assistance during emergencies or challenging behaviors when necessary.
