Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Educate patients and/or their families regarding care, medications, and home safety. Supervise and provide clinical direction to the HHA/STNA/LVN staff. Monitor assigned cases to ensure compliance with third party payer requirements.
