Hospice Social Worker (MSW) – Wyoming Valley, PA
Serenity Hospice PA
Serenity Hospice is seeking a compassionate and dedicated Hospice Social Worker to join our team serving the Wyoming Valley, Pennsylvania area. This is a rewarding opportunity to make a meaningful impact by supporting patients and their families during one of life’s most important journeys.
Position Summary:
The Hospice Social Worker provides emotional, psychosocial, and practical support to patients and families, helping them navigate end-of-life care with dignity and comfort.
Responsibilities:
Provide psychosocial assessments and ongoing support to patients and families
Assist with care planning as part of the interdisciplinary team
Connect patients and families with community resources
Offer counseling related to end-of-life issues, grief, and coping
Maintain accurate and timely documentation
Qualifications:
Master’s Degree in Social Work (MSW) required
Current Pennsylvania licensure (LSW/LCSW preferred)
Previous hospice or healthcare experience preferred
Strong communication and organizational skills
Valid driver’s license and reliable transportation
