Highland Manor nursing and rehabilitation center in Exeter , PA is a skilled nursing facility, providing clinically sophisticated sub-acute care and rehabilitation, as well as comprehensive nursing care. Coordinates medically related social services to nursing home residents.
Duties include:
Care planning
Discharge planning
Advocating for residents’ rights
Providing emotional support to residents and their family members.
Qualifications:
Must possess an associate’s degree in a human service field.
Must be Certified Social Worker in Pennsylvania
Experience working in a nursing home setting preferred.
