Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care. MINIMUM REQUIRED QUALIFICATIONS – TX/compact RN license BLS 2 years case management experience in an acute hospital setting Acute hospital experience during the past year with no more than a 90-day break LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8:00am-4:30pm with rotating weekends SYSTEMS – Meditech START DATE – 9/14/2026 Facility Location From the dramatic views of the Franklin Mountains to the breathtaking scenery of the Rio Grande, there is a lot to take in when visiting El Paso.
