Position: RN - Case Management

Shift Details

This is a full-time contracted position with a day shift occurring from 8:00 AM to 4:30 PM. The role requires 5 eight-hour shifts per week with rotating weekends. There is no on-call requirement.

Job Responsibilities

• Case Manager: Provide case management services for both inpatient and observation patients. Perform comprehensive assessments of psychosocial, medical, and discharge needs. Coordinate and drive the discharge plan by collaborating with the multidisciplinary team. Evaluate progression of care using InterQual throughout the care episode.
• Social Worker: Collaborate with RN case managers and the multidisciplinary team to perform assessments and develop appropriate discharge plans for high-risk patients. Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to address emotional, social, and financial aspects of care.

Qualifications

• Required Skills:
  
  
  • Case Manager: Minimum of 2 years of acute case management experience
  • Social Worker: 1-2 years of acute social work experience (licensed in TX)
• Preferred Skills: Experience with case management and social work, and possession of a Master of Social Work (MSW) degree.
