Job Description & Requirements RN Case Manager StartDate: 9/14/2026 Pay Rate: $1800.00 - $2500.00 POSITION SUMMARY – RN Case Manager POSITION DUTIES – Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care. Minimum Required Qualifications TX/compact RN license BLS 2 years case management experience in an acute hospital setting Acute hospital experience during the past year with no more than a 90-day break LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8:00am-4:30pm with rotating weekends SYSTEMS – Meditech
