Network ESC - NYC Based Medical Provider is in need of a Care Manager Registered Nurse. Monday Through Friday 8am-5pm Five years minimum of clinical experience as a Registered Nurse. Leadership experience preferred. Experience with Care Coordination, Utilization review and discharge planning Computer literacy required. Minimum 2 years’ prior experience in a health care setting, Care Management, or Managed Care setting required Ability to proficiently read and interpret medical records, claims data, pharmacy, lab reports and prescriptions required Ability to work closely with member and caregiver Education: 1. RN Licensed as a Registered Nurse in the State of New York required. 2. Baccalaureate degree required. 3. PRI and Screen certification within three months of employment is required. Master’s Degree and Bilingual Spanish Certification in Case Management preferred. Registered Nurse with current NYS license Bachelor’s Degree required #J-18808-Ljbffr
