Registered Nurse (RN) - Home Health Case Management

We are seeking a Registered Nurse (RN) Home Health Case Manager to oversee patient care in a home setting. Responsibilities: Manage a caseload of home health patients, supervise LPN visits, coordinate with therapy teams, conduct assessments, and develop personalized care plans aligned with physician orders. Support patients managing conditions such as CHF, COPD, diabetes, wounds, and medication regimens. Qualifications: Missouri or Compact RN license, valid driver’s license, and strong organizational, clinical, and communication skills. Key Duties: Build and manage patient care plans, supervise visits, ensure quality care, and collaborate with multidisciplinary teams.
