Contract | Days | 7:00 AM – 3:30 PM

Key Responsibilities

• Evaluate and treat patients with speech, language, and swallowing disorders in a long-term care setting
• Develop and implement individualized treatment plans based on accurate patient assessments and prognosis
• Communicate effectively with patients, medical staff, and interdisciplinary care teams
• Document patient progress and maintain records using Epic/EMR software
• Apply knowledge of the PDPM reimbursement system to support appropriate care planning and documentation

Required Qualifications

• Graduate of a college or university accredited by the American Speech and Hearing Association with a degree in Speech Language Pathology
• Current New York State license as a Speech-Language Pathologist
• Eligible for or holds the Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP) from ASHA
• Long-term care (LTC) experience required
• Candidates living within 75 miles of the facility are welcome to apply; local rates will apply

Preferred Experience / Skills

• Proficiency with MS Office and Epic/EMR software
• Working knowledge of the PDPM reimbursement system
• Strong assessment skills with the ability to guide patients toward achieving their fullest potential
