Requirements Specific Coverage Dates needed. Please only submit if the provider can cover all of these.7/1, 7/22,7/29.- potentially more in August. Will also need 1 day during the following weeks below: Nov 23-27Dec28-Jan 1Golisano Childrens Hospital :Peds physician- ophthalmology needed for up to 8 weeks a year to mainly cover staff vacations. Need is for both inpatient and ambulatory Days and weekend coverage needed- all onsite coverage Ambulatory- would work 8hrs on Tuesday and on Wednesday can work PRN if ROP treatment is needed. Inpatient- Wednesday - Rounding in NICU for ROP 7-1 followed by afternoon clinic IF treatment/f/u for ROP requiredsee attached detailed department specifics in this position. Name Clearance Requirements:Updated CVLee Health Signed AttestationOrientation is capped at 4 hrs.Onsite hand hygiene demonstration and employee health requirements mandated to be completed prior to first shift
