Job Title: Senior Children's Mental Health Targeted Case Manager

Reports To: Supervisor, Case Management

FLSA Status: Exempt

Purpose Statement

The Senior Children's Mental Health Targeted Case Manager provides comprehensive case management services to children with severe emotional disturbances (SED) and their families. The role supports families in accessing and coordinating mental health, educational, medical, social, vocational, recreational, legal, transportation, advocacy, and other community-based services to promote stability, well-being, and positive outcomes.

Responsibilities include completing or coordinating comprehensive diagnostic assessments (when needed), conducting functional assessments, developing Individual Family Community Support Plans (IFCSPs), coordinating services across systems, and ensuring continuity of care. Senior Case Managers continuously monitor and evaluate the effectiveness and appropriateness of services while advocating for the needs of children and families.

In addition to direct service responsibilities, Senior Case Managers serve as experienced leaders within the department by supporting quality assurance initiatives, mentoring colleagues, strengthening community partnerships, and providing operational coverage for Lead and Coordinator roles as needed.

Essential Functions

• Maintain an active case management caseload while balancing senior-level responsibilities. Expected case management FTE ranges from approximately 50% (nine clients) to 90% (17 clients), depending on departmental needs.
• Complete comprehensive assessments, reassessments, service planning, care coordination, and ongoing monitoring in accordance with Minnesota Children's Mental Health Targeted Case Management standards.
• Build collaborative, strengths-based relationships with children, families, providers, schools, and community partners.
• Coordinate services across multiple systems to ensure continuity of care and successful implementation of treatment and support plans.
• Monitor client progress, identify barriers, reduce risk factors, and support measurable growth and positive outcomes.
• Conduct peer audits to support quality assurance and regulatory compliance.
• Model best practices in case management, ethical service delivery, documentation, and team collaboration.
• Mentor and support case managers through coaching, consultation, and training.
• Build and maintain strong partnerships with community providers and stakeholders.
• Provide coverage and operational support for Lead and Coordinator functions as assigned.
• Participate in program development, quality improvement initiatives, and other departmental projects.

Qualifications

Education and Experience

• Bachelor's degree in one of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university.
• Employed continuously as a Children's Mental Health Targeted Case Manager at Youable Emotional Health for one (1) or more years.
• Two (2) years of experience as a Case Manager.

Knowledge, Skills and Abilities

• Proficient knowledge and understanding of mental health issues and resources.
• General knowledge and understanding of case management services.
• Proven ability to establish and maintain positive rapport with a wide variety of clients and professionals.
• Demonstrated ability to identify and resolve problems independently in a variety of arenas with both clients and professionals within the community.
• Demonstrated excellent verbal and written communication skills.
• Demonstrated ability to collaborate and utilize the skills of the program team and other staff.
• Proven ability to utilize client strengths to promote positive change.
• Proficient computer skills in Microsoft Office Suite and Carelogic EHR.
• High degree of organization and ability to work independently.
• Ability to manage time efficiently in order to ensure paperwork and billing are on time and completed correctly.

Physical Demands/Environmental Factors

• Exposure to fragrances and other environmental elements that may affect a chemical sensitivity.
• Local travel within the metro area.
• Availability to work up to two evenings per week.
• Must have a valid driver's license, working vehicle, and auto insurance.
• Visits and services provided in a variety of arenas, including in-home, in the community, and in schools.
• Interacting with a range of personalities and diverse individuals.
• Sitting for extended periods of time.
• Ability to lift up to 25 pounds.
