Registered Nurse Ambulatory (FT-Day, 40hrs/week) - Oral Surgery - Abington, PA

Qualifications and Requirements Graduate of an accredited school of nursing required; BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred. 1+ years’ experience as a Nurse, in an ambulatory setting preferred. Current RN licensure in the state of practice. BLS required based on organizational guidelines/policies. Encouraged to obtain ambulatory care nursing certification. Expected to participate in or support quality, evidence-based practice and research of nursing governance initiatives. Responsibilities Use the nursing process in patient- and family-centered care, valuing the uniqueness of each individual and addressing physical, psychological, emotional, and social needs of patients. Collaborate with primary care clinical team, providers, and support staff to provide clinical nursing interventions. Communicate effectively and respectfully with patients to address individualized needs and priorities consistent with their cultural beliefs and values. Integrate evidence-based practice, clinical expertise, patient and family preferences into delivery of optimal healthcare. Collaborate and communicate with primary care providers regarding patient care and management; support efficient and timely communication between providers and specialists. Support implementation of clinical care pathways for common primary care conditions and participate actively in quality improvement opportunities. Utilize evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication. Provide patient and family education for preventive care, chronic disease management, medication management, and management of common illnesses treated in the primary care setting. Document all patient interactions and clinical care in electronic medical record (EMR) according to organizational standards and compliance with applicable regulations. Perform care coordination for patients (e.g., abnormal results, triage, RN verification of vaccinations and other injections, management of in-baskets such as prior authorizations and forms). Oversee clinical care essential to assessment, promotion, maintenance, and restoration of patients’ health and wellness. Minimize risk of harm to patient, family by following established nursing practices and protocols. Use information technology to communicate, manage knowledge, and mitigate errors. Use data to monitor outcomes of care processes and support ethical decision-making; use improvement methods to design and test changes to continuously improve quality and safety of patient care. Function effectively within primary care inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care. Recognize patient and family in providing safe, compassionate, and coordinated care based on respect for patient’s preferences and cultural beliefs. Create an environment of belonging and inclusion and demonstrate skills in problem solving and conflict resolution. Demonstrate commitment to learning and seek opportunities for professional growth and development. Actively participate in team huddles/assemblies to provide announcements and clinical operations updates. Demonstrate willingness to give and receive feedback and integrate appropriate changes into practice.
