Work Environment

This position is primarily onsite, involving direct patient interactions in various home settings throughout West Palm Beach. The work environment is dynamic, allowing for flexibility with the schedule while also fostering a supportive atmosphere amongst staff. The typical work hours are Monday through Friday, 9 AM to 5 PM. As a home care physical therapist, you will manage a varied caseload while ensuring a high standard of care for each individual patient.

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, assessing their mobility and functional limitations to develop customized treatment plans.
• Deliver one-on-one therapeutic services focusing on manual therapy techniques, exercise interventions, and patient education.
• Maintain a manageable patient load to provide dedicated attention to each individual, ensuring the best possible outcomes.
• Create and adjust evidence-based treatment plans that emphasize active rehabilitation, guiding patients through effective exercise regimens.
• Document patient progress meticulously using EMR software, particularly Web PT, to maintain comprehensive and accurate medical records.
• Communicate effectively with patients and caregivers to foster an understanding of the treatment process and encourage adherence to prescribed home exercises.
• Monitor and evaluate patient responses to treatment, making necessary adjustments to optimize recovery and healing.
• Educate patients on injury prevention strategies and lifestyle modifications to promote long-term wellness.
• Collaborate with physicians, occupational therapists, and other healthcare professionals involved in patients' care to ensure unified and coordinated treatment approaches.
• Engage in ongoing professional development to stay current with advancements in physical therapy treatments and methodologies.

Patient Population / Client Focus

The Physical Therapist will work primarily with outpatient individuals recovering from orthopedic injuries, post-surgical rehabilitation, chronic pain, and other musculoskeletal conditions. Our approach is holistic, taking into consideration the whole person, their lifestyle, and personal goals, ensuring treatments are tailored to meet their specific needs.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) program from an accredited institution.
• A valid and active Physical Therapist license in the state of Florida, with no history of sanctions or board proceedings.
• Deep understanding of manual therapy techniques and a proven ability to develop effective exercise programs tailored to individual needs.
• Proficiency in utilizing EMR software, such as Web PT, enhancing the efficiency and accuracy of documentation and patient tracking.
• Strong communication and interpersonal skills, enabling effective patient interactions and collaboration with interdisciplinary teams.

Preferred Experience

• Previous experience in home care physical therapy settings or outpatient clinic environments.
• Familiarity with current best practices in physical therapy, particularly for orthopedic conditions.
• Knowledge and application of evidence-based practice guidelines for effective treatment delivery.
• Experience in developing specialized fitness and wellness programs based on patient assessments and goals.

Compensation and Benefits

Full Motion Physical Therapy offers a competitive salary ranging from $100,000 to $120,000 annually, based on experience and qualifications.
