Job Type : Full-Time | On-Site

Schedule: Monday-Friday | 8:00 AM - 5:00 PM

Position Overview :

We are seeking a compassionate and patient-focused Primary Care Physician to join a collaborative healthcare team serving adult and senior patient populations. This position offers a value-based, team-oriented care model focused on delivering high-quality outcomes, preventative care, and comprehensive patient management.

Key Responsibilities:


• Provide comprehensive primary care services including evaluation, diagnosis, treatment, and follow-up care.

• Manage and coordinate patient care plans with interdisciplinary healthcare teams.

• Focus on preventative care, chronic disease management, and improving patient outcomes.

• Coordinate specialty referrals, home health services, durable medical equipment, and post-acute care transitions.

• Maintain accurate and timely medical documentation in compliance with regulatory standards.

• Collaborate with physicians, advanced practice providers, behavioral health professionals, pharmacists, and care coordinators.

• Participate in team meetings, quality improvement initiatives, and patient care planning discussions.

• Ensure compliance with clinical protocols, payer guidelines, and healthcare regulations.

Requirements:


• MD or DO from an accredited medical school.

• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.

• Active and unrestricted medical license (or willingness to obtain licensure in Mississippi).

• Active DEA license.

• Medicare and Medicaid provider enrollment preferred.

• Strong communication, teamwork, and patient-centered care skills.
