Now Hiring: RN Care Manager CommWell Health is seeking a compassionate, organized, and clinically strong Registered Nurse Care Manager who thrives in a mission-driven environment and is passionate about coordinating care for patients living with multiple chronic conditions . In this role, you’ll work as part of a multidisciplinary team focused on population management, helping patients improve outcomes through education, care planning, and strong transitions of care support. If you’re a nurse who enjoys building relationships, problem-solving barriers to care, and helping patients navigate the healthcare system, this may be the perfect fit. What You’ll Do Identify patients who meet criteria for the care management program Review charts to assign acuity levels and assign patients to the appropriate care team Provide oversight to Care Coordinators completing comprehensive assessments and assisting with development of individualized Care Plans Develop and update care plans in collaboration with the patient, PCP, and care team Support transitions of care, including medication reconciliation, follow-up, specialist referrals, barriers to care, and education Coordinate communication with community partners such as hospitals, emergency departments, and discharge planners Support PCMH implementation and help teams understand care coordination workflows Monitor outcomes, identify gaps, and recommend improvements Required Graduate of an accredited RN program Current North Carolina RN License 2+ years of experience in primary healthcare, ambulatory care and/or home health within the last 5 years HS Diploma or GED Comfortable navigating an electronic health record (EHR) Valid NC driver’s license + clean driving record (approved for driving privileges) Preferred Experience with underserved or rural populations Spanish speaking preferred (not required) Why Join CommWell Health?
