Aging Care Manager 1 (Local Government) Wayne County Area Agency on Aging

Located in Honesdale, PA, Wayne County Area Agency on Aging is seeking a qualified individual for an exciting position assisting senior citizens with accessing resources to promote safety, independence, and well-being. This Aging Care Manager 1 role is ideal for a patient, compassionate, and organized candidate who enjoys working with people. Work begins as structured program of formal and informal training designed to develop the knowledge, skills, and abilities required for the professional aging care management services we provide. This role supports older adults from first contact through ongoing care management. The work includes specialized responsibilities and coordination with many partners. As an Aging Care Manager 1, you will perform the following duties: Case Management: Manage a caseload by guiding older adults and their families through needed services Service Coordination: Work with physicians, community programs, financial institutions, and other partners to support consumer needs Specialized Support: Carry out important duties such as Protective Services, Care Transitions, and Person-Centered Counseling Task Organization: Handle multiple assignments by completing paperwork, tracking details, and meeting required deadlines Travel Requirements: Travel outside the county when needed, including occasional overnight trips Work Schedule and Additional Information: Full-time employment Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch. On-call hours will be required, as needed. Telework: You will not have the option to telework in this position. Salary: In some cases, the starting salary may be non-negotiable. QUALIFICATIONS

Minimum Experience and Training Requirements Three years as an Aging Case Aide 2; or A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training. Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree. You must be able to perform essential job functions. Legal Requirements You must pass a background investigation. A conditional offer of employment will require a drug screening. This position falls under the provisions of the Child Protective Services Law.
