Care Coordinator

Summary:The person in this position will be responsible to act as the primary consumer advocate for the program assigned. The person in this position keeps abreast of progress, and coordinates all services delivered to the consumer and family internally and through external referral.Essential Job Functions:Administer assessments as neededConduct Needs AssessmentDevelop Service Plans in collaboration with the consumer and familyConduct monthly Service Plan reviewsDocument all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedureConduct individual case management sessionsProvide vocational education and skills building to the consumer as neededPerform activities on behalf of the consumer and family, including attendance at external appointments (e.g., court appearances, school staffing, etc.), as neededProvide referral and linkage for the consumer and family, as neededPrepare monthly Progress Reports, as required, for assigned consumers for courts, probation officers, caseworkers, etc.Coordinate consumer and family care with the treatment team and with external providersResponsible for the development of the discharge plan in coordination with other treatment team membersArrange transportation for consumers, as necessaryProvide case management to additional populations, as neededParticipate in case conferenceUnderstands HIPAA, Professional Ethics, Consumer Rights and ConfidentialityCompletes all orientation and mandated training activities, including E-Learning Courses, within required timeframes, and in accordance with the criteria established for this position.Participate in performance improvement activitiesEmbrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.Perform any other duties as assigned.Essential Qualifications:Certifications/Licensures:Certification as a certified case manager in good standing and recognized by the Florida Certification Board is preferred at time of hire; Preference will be given to those candidates who are currently Certified;Education:Graduation from an accredited bachelors program with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is requiredExperience and Competencies:Two years of previously supervised work experience in providing individual/family therapy is preferredBilingual is preferred.Must have reliable means of transportationMust have familiarity with Miami-Dade County.Maintains confidential files and records.The ability to identify and evaluate an individual’s strengths, barriers, problems, and needs for the development of a Wellness and Recovery PlanThe ability to evaluate the impact of substance use and mental health problems on the consumer’s functioning in major life areasKnowledge of the inter-relationship among substance use, mental health, and primary care disorders.The ability to conceptualize cases within a developmental framework for the purpose of treatment planning, intervention, and discharge planningKnowledge in the understanding of programs built on proven methods that ensure effective treatmentDemonstrate proficiency is the administration and interpretation of assessments, as neededThe ability to coordinate and integrate multiple services, as well as interface and communicate with internal and external providersThe ability to provide information to individuals and families concerning alcohol or other drug use, mental health problems, and the available services and resourcesAbility to use trauma informed care principles including ensuring consumer safety, preventing/avoiding re-traumatization, maximizing trustworthiness, enhancing consumer choice, maximizing collaboration, and emphasizing empowermentAbility to identify and describe key signs, symptoms, impact and manifestations of traumaAware of the differences between trauma informed and trauma-specific servicesUnderstands the bidirectional relationships among substance use and mental disorders and traumaUnderstands the differences among various kinds of abuse and trauma, including: physical, emotional, and sexual abuse;
