Hybrid Care Navigator: Patient Advocate & Care Coordination
Fallon Community Health Plan, Inc.
Fallon Health in Massachusetts is seeking a Navigator–Care Management to join an interdisciplinary team. You will coordinate care, conduct member assessments, and support care plans to improve access and outcomes for NaviCare members.
You will educate members, schedule services, and help address barriers to care, working closely with the Nurse Case Manager and providers to ensure seamless transitions and high-quality care.
