PT - Physical Therapist

Outpatient Physical Therapist - 40 Hours/Week

We are seeking a dedicated Outpatient Physical Therapist to evaluate assigned patients and develop, implement, and modify treatment plans tailored to their individual needs and goals, following physician direction. You will be responsible for the coordination and delivery of therapy services, ensuring functional treatment programs are designed for effective community reintegration. As an educator, you will guide patients, families, students, and coworkers, while upholding our organization's Mission Statement and Core Values.

This is a 40 hours per week position, with evaluations lasting 60 minutes and follow-up sessions every 30 minutes.

Responsibilities include:

• Conduct comprehensive evaluations of assigned patients and create individualized treatment plans.
• Document care provided in compliance with organizational guidelines and state regulations.
• Implement therapy interventions aimed at facilitating progress towards discharge.
• Participate in the oversight and development of students assigned to the program.
• Maintain professional licensure and fulfill state-mandated continuing education requirements.
• Engage in performance improvement initiatives to enhance quality of service delivery.
• Ensure adherence to safety protocols and promote a safe environment for patients and employees.

Required experience/certifications/licensure include:

• Degree in Physical Therapy from an accredited institution.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• Valid and active Physical Therapist license in the state of practice.
