Social Worker - Case Management

Social Worker - Case Management Children's Hospital of Michigan is a leading pediatric hospital. This role supports patients and families by coordinating care and resources throughout the continuum of care. Summary Description The Social Worker is responsible for facilitating care and coordinating resources to help patients achieve optimal health, access to care, and appropriate utilization of resources. The individual assesses transition needs, conducts psycho‑social assessments, promotes timely throughput, and prevents readmissions while respecting patient self‑determination. Integrate patient assessment, choice, and available resources to create a successful transition plan. Handle complex psycho‑social assessments and plans for high‑risk patients. Provide psycho‑social assessment and intervention for patients with behavioral health, financial barriers, end‑of‑life, or medication adherence needs. Delegate implementation of the transition plan to LVN/LPN or assistant staff and follow up for completion. Ensure all plan elements are implemented and communicated to the healthcare team, patient/family, and post‑acute providers. Provide patients with information to make informed choices about community services per Tenet policy. Complete Final Discharge Disposition Form Assessment for Medicare patients per Tenet policy. Document thoroughly in the Tenet Case Management system (40% of daily responsibilities). Care Coordination Screen patients for factors that may affect progression of care and intervene to promote timely throughput. Conduct assessments and stratify patients at risk for readmission or needing case management services. Assist with adoption/abuse/neglect cases and report to local, state, and/or federal agencies. Ensure the plan of care aligns with patient choice and available resources. Communicate patient needs and ensure the healthcare team is mutually accountable to achieve the plan of care. Collaborate with physicians, nurses, ancillary staff, payors, patients, and families to achieve optimal outcomes (40% daily). Education Ensure and provide education to patients, physicians, and the healthcare team regarding safe and timely transition. Provide education about resources and benefits available to patients and the economic impact of care options. Confirm education has been provided to patient/family/caregiver by the healthcare team prior to discharge (10% daily). Compliance Ensure compliance with federal, state, and local regulations and accreditation requirements impacting case management scope. Adhere to department structure, staffing, policies, and procedures to comply with CMS conditions of participation and Tenet policies. Operate within the Social Work scope of practice as defined by state licensing regulations (10% daily). Qualifications Minimum Qualifications Master’s degree in Social Work from a Michigan Board‑approved program accredited by the Council on Social Work Education. Current license as a Licensed Master’s Social Worker in Michigan, or a limited license with full licensure within three years. Two years of acute hospital experience preferred. Demonstrate competency in using the Tenet Case Management documentation system within 30 days of hire. Attend hospital and department orientation covering Tenet case management and compliance policies. Accredited Case Manager (ACM) preferred. Skills Required Analytical, critical thinking, and problem‑solving skills to identify and address improvement opportunities. Interpersonal skills to work productively with patients, families, and all levels of hospital personnel. Verbal and written communication skills for diverse populations. Ability to manage stress, multiple priorities, and complex medical/emotional cases. Teaching abilities for educational programs for staff, patients, families, and community. Organizational skills to lead and coordinate activities in a fast‑paced environment. Knowledge and physical ability to assess patients/families for psychosocial health status and needs. Computer literacy to utilize case management systems.
