In this role, you will help patients regain mobility, improve strength, manage pain, and restore independence — all from the comfort of their homes. What You’ll Do As a Physical Therapist, you will: Perform comprehensive patient evaluations and develop individualized treatment plans Assess functional limitations and establish appropriate therapy goals Deliver skilled therapeutic interventions to improve mobility, strength, balance, and overall function Monitor patient progress and modify treatment plans as needed to maximize outcomes Educate patients and caregivers on exercises, safety, and ongoing care strategies Maintain timely, accurate, and compliant clinical documentation Collaborate with physicians, nurses, case managers, and interdisciplinary care teams Follow infection control and universal precaution standards during patient care Support the agency’s mission of providing compassionate, high-quality home health services Qualifications Graduate of an accredited Physical Therapy program Current and unrestricted Physical Therapist license CPR Certification required Minimum of 1 year of home health experience preferred HomeCare HomeBase experience preferred Current Driver’s License and reliable transportation Valid Auto Insurance Physical Requirements Ability to stand, walk, bend, squat, kneel, and climb for extended periods Ability to lift, push, pull, or carry up to 100 lbs. #J-18808-Ljbffr Choice Health at Home
