WHAT YOU'LL DO

• Conduct patient assessments and develop individualized care plans
• Coordinate services with physicians, therapists, caregivers, and community

resources

• Monitor patient progress and adjust care plans as needed
• Educate patients and families on medications, treatment plans, and wellness

strategies * Maintain accurate clinical documentation and regulatory compliance

QUALIFICATIONS

• Current Minnesota Registered Nurse (RN) license required
• Home health, case management, transitional care, or related clinical

experience preferred * Strong assessment, communication, and care coordination skills
