Compensation: $30-$37 per hour

Schedule: Part-Time to Start | Weekend, On-Call, and Holiday Rotation

Location: Greater Twin Cities Metro Area | Field-Based Position

As a Hospice LPN, you'll be an essential member of the care team, working alongside experienced RNs, physicians, social workers, chaplains, and aides to deliver personalized hospice care. Your observations, communication, and clinical judgment help shape every patient's plan of care.

What You'll Do

• Provide skilled nursing care including medication administration, wound care, symptom management, and patient monitoring.
  
• Identify and communicate changes in patient condition to the RN Case Manager and interdisciplinary team.
  
• Educate patients, caregivers, and families about medications, comfort measures, and hospice services.
  
• Deliver care in homes, assisted living communities, memory care, and skilled nursing facilities.
  
• Complete timely EMR documentation and participate in weekend, holiday, and on-call rotations.
  

What You'll Bring

• Active Minnesota Licensed Practical Nurse (LPN) license.
  
• Previous experience in hospice, home health, long-term care, skilled nursing, oncology, or palliative care .
  
• Excellent communication and interpersonal skills.
  
• Ability to work independently while collaborating with the interdisciplinary team.
  
• Compassion, professionalism, and strong clinical judgment.
