Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team. Contribute to the development and ongoing refinement of an age-appropriate, culturally relevant plan of care, engaging the resident, family, and healthcare team to achieve outcomes. Recommend revisions to the plan of care based on evaluation of resident outcomes and coordinate referrals as needed to address all aspects of care. Maintain current knowledge and competency in nursing practice; participate in mentoring or orientation of staff; promote safety and quality in care delivery. Demonstrate leadership under the direction of the RN and ensure adherence to policies, procedures, and safety standards; manage exposure risks consistent with facility protocols. Requirements Active New York LPN license. BLS certification (American Heart Association or RQI acceptable). Minimum 1 year of LPN experience, preferably in a long-term care setting; ability to work with adult and geriatric populations. Willingness to work holiday coverage and participate in day/evening/night rotation as needed. Ability to follow established policies and collaborate with the Interdisciplinary Team in a fast-paced environment.
