Overview
Licensed Practical Nurse | Long Term Care | Geneva, NY. Travel assignment starting 09/15/2026 for 24 weeks. Weekly pay range $1,295 per week to $1,315 per week. Days shift. Setting: Skilled Nursing Facility. This temporary assignment is managed through Healthforce Healthcare LLC.

Responsibilities
- Provide direct and indirect skilled nursing care for assigned adult and geriatric residents under the supervision of an RN.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the interdisciplinary team.
- Contribute to the development of an age-appropriate, culturally relevant plan of care, involving the resident, family, and healthcare team, and update as outcomes are evaluated.
- Recommend revisions to the plan of care as needed and coordinate referrals to ensure comprehensive resident needs are addressed.
- Maintain current knowledge and clinical competency; participate in mentoring and inservice activities; uphold safety and quality standards in all patient care.
- Demonstrate leadership in the professional practice setting, collaborate with physicians and other staff, and provide care aligned with geriatric patient needs.

Requirements
- Active New York LPN licensure
- Basic Life Support (BLS) certification (AHA or RQI)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to provide care to adult and geriatric residents
- Ability to work the listed shift (days) and function effectively under RN supervision in a skilled nursing environment
