If you have any questions, please escalate to the PSJH admin team. • Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active only • DEA Required: Yes • Boards: BC only • Skills/Certs: ACLS, BLS PLEASE ENSURE AN UPDATED CV WITH ALL HOSPITAL AFFILIATIONS IS UPLOADED DURING PRESENTATION. Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties. ________________________________________________________________ ADDITIONAL DETAILS inpatient/rounding in Cardiology within the last 2 years required. We are looking for one provider to cover a Monday-Friday schedule.
