About the Role The Home Care Coordinator helps ensure clients receive high-quality, consistent care and exceptional service. Serving as a key liaison between clients, families, caregivers, and the Home Care team, this position coordinates services, supports client onboarding, conducts supervisory visits, monitors satisfaction, and helps maintain individualized care plans. Schedule: Full-Time | Monday–Friday, 8:30 a.m.–4:30 p.m. Pay: $22/hour What You’ll Do Coordinate the onboarding of new clients, including welcome calls, service orientations, and communication with clients and families. Assist with client assessments and the development, implementation, and review of individualized care plans. Schedule and conduct supervisory visits to monitor service quality and client needs. Monitor client satisfaction and address concerns to support a positive experience. Provide compassionate support and guidance while treating every client with dignity and respect. Communicate effectively with clients, families, caregivers, and team members to promote continuity of care. Support the Home Care Manager with daily operations and client needs. Participate in the department’s on-call rotation as assigned. What We’re Looking For Associate degree or equivalent experience in healthcare, senior services, social services, or a related field. At least one year of experience in home care, healthcare, senior living, case management, client services, or a related field. Experience working with older adults; experience with individuals living with dementia is preferred. Valid Illinois driver’s license with a good driving record. Strong communication, problem-solving, and decision-making skills. Ability to work independently while collaborating effectively with a team. Strong organizational skills and the ability to manage multiple priorities. Computer proficiency. CPR and Food Handler certifications required within 30 days of hire.
