Cridersville Health Care Center is entering an exciting new chapter, and we are looking for an experienced, driven, and compassionate Director of Nursing (DON/RN) to help lead the way.

We are currently seeking a Director of Nursing (DON/RN) to join our leadership team.

Job Responsibilities and Duties:

• Provide overall leadership and direction for the facility's nursing department.

• Ensure residents receive high-quality, compassionate nursing care in accordance with their individualized plans of care.

• Recruit, develop, mentor, and retain a strong team of nursing professionals.

• Establish a culture of accountability, teamwork, communication, and clinical excellence.

• Provide hands-on leadership and maintain a visible presence throughout the facility.

• Oversee nursing staffing and scheduling to maintain appropriate coverage while effectively managing labor and agency utilization.

• Monitor resident outcomes, clinical indicators, hospitalizations, falls, wounds, infections, and other quality measures.

• Identify areas of clinical opportunity and develop effective plans of correction and performance improvement initiatives.

• Ensure compliance with all federal, state, and local regulations governing skilled nursing and long-term care.

• Lead nursing preparation for state surveys, audits, and regulatory visits.

• Participate in Quality Assurance and Performance Improvement (QAPI) activities.

• Review clinical documentation for accuracy, completeness, and regulatory compliance.

• Ensure physician orders, medications, treatments, and changes in condition are appropriately communicated and documented.

• Collaborate closely with the Administrator, Medical Director, department leaders, physicians, residents, and families.

• Investigate clinical concerns, incidents, grievances, and allegations and ensure appropriate follow-up.

• Provide education, coaching, and ongoing development to nursing leadership and frontline clinical staff.

• Support infection prevention, risk management, and resident safety initiatives.

• Maintain nursing policies, procedures, and standards consistent with regulatory requirements and organizational expectations.

• Serve as a clinical resource and leader during challenging situations and periods of operational change.

• Promote resident-centered care while fostering an environment where employees feel supported, valued, and accountable.

Qualifications:

• Current Registered Nurse (RN) license in the State of Ohio required.

• Previous Director of Nursing, Assistant Director of Nursing, or comparable nursing leadership experience strongly preferred.

• Skilled nursing facility (SNF) and/or long-term care experience strongly preferred.

• Strong knowledge of state and federal regulations governing skilled nursing facilities.

• Demonstrated ability to lead, develop, and retain nursing teams.

• Strong clinical assessment, critical-thinking, and problem-solving skills.

• Experience with survey preparation, plans of correction, quality improvement, and clinical compliance preferred.

• Excellent communication, organizational, and leadership skills.

• Ability to lead from the floor and remain engaged with residents, families, and staff.
