Home Health Physical Therapist - West and NW Metro Areas

Namaste Health has seen record growth on its Home Health service line over the last 6 months, grabbing a significantly higher market share in the metro area while maintaining a very high employee retention rate. The company is actively seeking at least one Full Time PT with the intention to grow into a full‑time position as quickly as possible. JOB SUMMARY The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. This position is compensated using a Per‑Visit model, with a routine visit rate between $60 and $70. Admissions, recertifications, evaluations and discharges are weighted higher. DUTIES & RESPONSIBILITIES Assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Provide patient, caregiver, and family counseling and education. Prepare clinical notes and progress summaries based on attainment of goals. Communicate with the physician who is responsible for the home health plan of care and other health care practitioners as appropriate. Provide physical therapy services to patients according to a written physician's plan of care, including but not limited to assessing therapeutic status, evaluating the home environment, assessing muscle strength, mobility, gait, ROM, and directing treatment. Instruct patients and families/caregivers in the use and care of therapeutic appliances. Determine priority needs for physical therapy and report to physician patient’s reaction to treatment or changes in condition. Initiate a physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of therapy, participating in case conferences and ensuring they understand program goals. May train patients in the use of prosthetic devices. Identify patient and family/caregiver needs for other home health services and refer as necessary. Participate in discharge planning and provide in‑service education programs for nursing organization personnel as needed. Participate in peer consultation processes. When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two (2) weeks; supervise Physical Therapy Assistants according to organization policy and state regulations. Participate in the HHA's quality assessment and performance improvement program and HHA-sponsored in‑service training. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) Licensed to practice as a physical therapist within the State, if applicable. Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master’s program in physical therapy approved by CAPTE or its successor organization. Two years of appropriate experience as a physical therapist; community/home health experience is preferred. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
