CareMore Health (now part of Mosaic Health) is seeking a Primary Care Physician (PCP) to provide comprehensive, integrated, and personalized healthcare services to patients of all ages in Upland, California. This role focuses on value-based care, emphasizing accountability for patient outcomes and collaborative efforts within a multidisciplinary team. The PCP will manage a panel of patients, particularly those with complex or chronic conditions, ensuring continuity of care and promoting overall patient well-being. Practice Info Location: Upland, California Focuses on value-based care and accountability for patient outcomes Multidisciplinary team collaboration, including advanced practice providers, case managers, and specialists Responsibilities Provide comprehensive care for a panel of patients with multiple co-morbidities, focusing on prevention, diagnosis, and treatment of acute and chronic conditions Prevent unnecessary ER visits and hospitalizations Identify high-risk members daily and discuss them during high-risk huddles Work closely with a multidisciplinary team to create and execute patient-centered care plans Educate patients and their families on wellness, prevention, healthy lifestyle choices, and the management of their health conditions Manage patient referrals to specialists, ensuring effective communication and coordinated follow-up care Maintain thorough and timely medical records using an electronic medical record (EMR) system Compensation Compensation: $232,506 to $348,758 This position is bonus eligible based on individual and company performance Requirements Required: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school Required: A valid, unrestricted California Medical Board license Required: A current Drug Enforcement Agency (DEA) certificate Required: Potentially a BLS/ACLS certification Required: Board certification or eligibility in Internal Medicine Required: Proficiency in managing complex and chronic conditions Required: Strong diagnostic and clinical skills Required: Excellent communication and interpersonal skills Required: A commitment to patient-centered care
