Remote RN Case Manager (Nationwide | Monday–Friday, No Weekends) Join a mission-driven team helping members achieve better health outcomes—100% remote. We’re seeking an experienced Registered Nurse (RN) Case Manager to provide telephonic and occasional face-to-face care coordination, guiding members through complex health needs with a holistic, proactive approach. Occasional late shift rotation until 8:00 pm No weekends or holidays Impactful work improving member health and outcomes Licensing support – additional state licenses fully covered What You’ll Do Assess, plan, and coordinate care to support members’ overall wellness Use clinical judgment and data to identify risks and remove care barriers Develop personalized care plans addressing complex and co-morbid conditions Collaborate with multidisciplinary teams to optimize outcomes Engage members through strong telephonic communication and coaching What You Bring - REQUIRED Active, unrestricted RN license (Compact license preferred) Willingness to obtain additional state licenses (company paid) 3+ years acute care RN experience (med/surg, ICU, or post-surgical) Strong assessment, communication, and critical thinking skills Travel of less than 5% may be required in the event of clinical audits Preferred Case management or home health experience (1+ year) Compact RN licensure CCM certification Proficiency with Microsoft Office tools Strong telephonic communication skills Education Associate’s Degree in Nursing required (BSN preferred) Make a difference—apply today and help shape better health journeys for members nationwide. Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
