RN (Registered Nurse) Case Manager

Schedule Monday – Friday, 8:30a-5p How You’ll Make a Difference As a Registered Nurse Case Manager (RNCM) you will provide one‑on‑one patient care and work at the top of your license. You’ll coordinate care and deliver home‑based nursing care directed by an attending physician. You’ll be part of an interdisciplinary team focused on compassionate, quality care and positive outcomes for patients. How You’ll Work You’ll assume ultimate accountability for assessing patient condition and creating the plan of care, case‑manage and provide clinical care under physician direction, educate patients and families, and ensure patient safety. You will work autonomously, managing your time efficiently and performing all duties within the state scope of practice. Major Areas of Responsibility Plan of Care: Observe and monitor patient conditions, perform OASIS assessments to develop and adjust individualized care plans. Minimize Patient Risk: Administer medication as prescribed and reduce re‑hospitalizations by prioritizing visits for high‑risk patients. Supervision: Oversee and supervise care provided by nurse aides and LPNs. Continuity of Care: Manage multidisciplinary care, promote continuity during admissions, transfers, and discharges. Family Educator/Advocate: Educate patients and families on the disease process using teach‑back methods. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines, collaborate with the interdisciplinary team and physicians. Rules and Regulations: Understand and follow agency policies and procedures, communicate schedule changes to schedulers or supervisors. Operations: Attend in‑service training and mandatory agency meetings. Hard & Soft Skills Compassionate communicator with a positive attitude. Patient, diligent, and detail‑focused. Observant, follows directions, and effective at prioritizing tasks. Requirements Registered Nurse with a current license in the state of employment. Minimum one‑year RN experience in an acute care setting. Home care experience preferred. Valid driver’s license and auto insurance in your name as a driver. Capable of meeting all physical demands of the job.
