About Company:
Redwood Coast Regional Center (RCRC) is one of 21 Regional Centers that are community-based, private non-profit corporations under contract with the California Department of Developmental Services. RCRC provides intake, assessment, diagnosis, and coordinates community-based services for over 10,000 children and adults with developmental disabilities in Del Norte, Humboldt, Lake, and Mendocino counties. We work in partnership with many individuals, family members, community leaders, and agencies to help the people we serve live quality, independent lives in our region.
About the Role:
The Service Coordinator plays a pivotal role in connecting individuals and families with essential social assistance services to improve their quality of life. This position involves assessing client needs, developing personalized service plans, and coordinating with various community resources and agencies to ensure timely and effective support. The Service Coordinator acts as a liaison between clients and service providers, advocating for client needs and facilitating communication to resolve any barriers to service delivery. Success in this role requires a compassionate approach, strong organizational skills, and the ability to manage multiple cases simultaneously while maintaining detailed records. Ultimately, the Service Coordinator contributes to the well-being and empowerment of vulnerable populations within the Lakeport community by ensuring access to comprehensive social support services.
Minimum Qualifications:
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Minimum of 2 years experience in social assistance, case management, or a related role.
- Strong knowledge of social service programs, community resources, and eligibility criteria.
- Excellent communication and interpersonal skills to effectively engage with diverse client populations.
- Proficiency in maintaining confidential client records and using case management software.
Preferred Qualifications:
- Master’s degree in Social Work or related discipline.
- Experience working within the Lakeport community or similar social service environments.
- Familiarity with local, state, and federal social assistance regulations and compliance requirements.
- Bilingual abilities to support non-English speaking clients.
Responsibilities:
- Conduct comprehensive assessments of client needs and eligibility for social assistance programs.
- Develop and implement individualized service plans in collaboration with clients and relevant agencies.
- Coordinate and monitor the delivery of services, ensuring clients receive appropriate support in a timely manner.
- Maintain accurate and up-to-date case records, documentation, and reports in compliance with organizational and regulatory standards.
- Advocate on behalf of clients to address barriers and facilitate access to community resources and services.
- Collaborate with multidisciplinary teams, including healthcare providers, social workers, and community organizations.
- Provide ongoing support and follow-up to evaluate client progress and adjust service plans as needed.
- Participate in community outreach and education initiatives to raise awareness of available social assistance programs.

The Service Coordinator utilizes strong communication skills daily to build trust and rapport with clients, ensuring their needs are clearly understood and addressed. Organizational skills are essential for managing multiple client cases, coordinating services, and maintaining detailed documentation accurately. Knowledge of social assistance programs and community resources enables the coordinator to connect clients with appropriate support efficiently.
