RN Case Manager - Hospice

This position includes a $5,000 sign on bonus that will be paid out over the course of the first year. This role covers mostly the West Metro area and would be based out of St. Louis Park, MN. Summ aryThe Registered Nurse (RN), Hospice provides end-of-life care to patients with a terminal diagnosis needing hospice services in their home or community setti ionment Education, Experience and Creden tialsGraduate of an accredited school of nursing requ ired.Must possess and maintain State of Minnesota RN licen sure.One year of Clinical Nursing experience prefe rred.Med-Surg, TCU, LTC, Assisted Living, Home Health, or Hospice experience prefe rred.Possess a valid driver’s license with good driving record, and proof of valid insurance. Must have reliable transporta tion.CPR Certification requ ired. Knowledge, Skills And Abi litiesExcellent interpersonal, collaboration, and relationship building skills to effectively work with a diverse group/variety of people and personalities, including developing relationships with vendors, family members, volunteers, clients and staff.Able to provide client care planning and client teaching s kills.Able to plan, prioritize, coordinate, and manage own work. Able to work unsupervised, make decisions and solve routine problems independently, effectively and creat ively.Work with a proactive, ownership approach to work and operate with a sense of ur gency.Understand and maintain confidential nature of organization and client information, including and in accordance with HIPAA regula tions.Able to assist residents with mobility needs such as transfers, ambulation, escorts or use of assistive devices. Physical Requirements Necessary To Perform T his Job Incumbent must be able to:Perform heavy work, lifting up to 50 pounds, and occasionally lifting/carrying of objects weighing up to 75 pounds.Push/pull clients in wheelchairs up to 200 pounds a distance of sever al feet.Vision acuity to read fine print, warning labels, doctor prescriptio ns, etc.Communicate clearly and effectively both orally and in writing, including possessing good English-speaking skills, fluency and understand ability.Must be able to successfully pass training and education modules, and exhibit competency with medication administration and docume ntation.Twist, turn, bend, stoop, kneel, squat and reach frequently in the performance of client assistance responsib ilities.Frequently stand and walk during an eight-hou r shift.Perform repetitive movements of the upper extr emities.Exert moderate physical energy for up to eight hours per day.Apply manual dexterity for using and maintaining tools and eq uipment.Work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.
