RN Care Manager — Elder Care & Community Coordination

Providence Health & Services is seeking a Registered Nurse Care Manager for the Providence PACE program in Washington. The role focuses on patient assessment, care planning, implementation, and coordination to ensure medically and financially appropriate care. You will work with the IDT Social Worker and Physician to collect medical and social information to support safe, well‑being decisions.
