Geriatric RN Care Manager — Care Coordination & Discharge
Providence Health Plan Group
Providence PACE is a Program of All-Inclusive Care for the Elderly aimed at helping older adults remain healthy in the community through clinics and home visits. The RN Care Manager will perform assessment, planning, implementation, coordination and monitoring of care, including coordination with IDT Social Worker and Physician to ensure safe, well-being outcomes for participants. The role operates Monday-Friday day shift with benefits, and a potential $10,000 hiring bonus for eligible external #J-18808-Ljbffr Providence Health Plan Group
