Key Responsibilities\n

• Conduct thorough assessments and evaluations to identify the mental health needs of patients, employing evidence-based practices tailored to individual situations.

• Maintain open lines of communication with supervisors, direct care staff, and family members, sharing updates on patient progress and treatment modifications.
\n
• Create and manage an organized schedule of appointments, home visits, and follow-up observations, ensuring timely and effective patient interventions.
\n
• Coordinate home visits and in-home observations diligently, documenting relevant findings and engaging in comprehensive chart reviews.
\n
• Regularly assess the implementation of therapeutic interventions, adjusting them as necessary to maximize patient engagement and success.
\n
• Actively participate in interdisciplinary team meetings, sharing insights and strategies to enhance collaborative care delivery.
\n
• Complete detailed progress notes on a monthly and quarterly basis, providing insight into treatment efficacy and patient's well-being.
\n
• Organize and lead training sessions to ensure staff are proficient in the application of our program’s policies and procedures, fostering a culture of continuous improvement.

• Conduct follow-up interviews and ongoing home visits to ensure patient needs are met, providing additional support as necessary.

Patient Population / Client Focus\n

This role primarily focuses on working with both adults and youth, addressing a wide spectrum of mental health issues including anxiety, depression, trauma, and substance use disorders. Your expertise in dealing with diverse backgrounds will enhance our ability to deliver effective care and facilitate individual growth and recovery.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Psychology, Counseling, or a related field.
\n
• Active and unrestricted licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in the state of California.
\n
• A minimum of two years of relevant counseling experience, ideally in a clinical or community health setting.
\n
• Demonstrated understanding of legal and ethical standards in mental health practice.
\n
• Exceptional collaboration skills, evidenced through successful cooperation with multidisciplinary teams.

Preferred Experience\n

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly desirable.

• Competitive salary ranging from USD 80,000 to 104,000 per year, commensurate with experience and caseload.
