Client Care Manager

ECO Supportive Living provides Medicaid waiver home-based services, private pay care, and veteran-focused support to clients across multiple Nebraska counties. Since 2021, we've built our reputation on compassionate, compliant, person-centered care, and our Client Care Managers are at the heart of it. About the Role As a Client Care Manager, you'll own a caseload of clients receiving home-based services and serve as the primary link between clients, families, caregivers, and our office team. This is a field-based role: you'll be out in the community conducting home visits, supervising care delivery, and making sure every client's plan of care is followed and documented. What You'll Do Manage an assigned caseload of clients, including regular home visits and care plan reviews Supervise and support caregivers in the field, including coaching and performance feedback Coordinate with service coordinators, families, and Nebraska DHHS as needed Maintain accurate, timely documentation in our care management system Monitor compliance with Medicaid waiver requirements, client rights, and agency policies Participate in caregiver onboarding, orientation, and ongoing training Respond to client and caregiver concerns with urgency and professionalism What We're Looking For Bachelor's degree in social work, human services, healthcare administration, nursing, or a related field required Experience in home care, case management, social services, or a related field (Medicaid waiver experience a strong plus) Strong organizational and documentation habits; compliance matters here Excellent communication skills with clients, families, and field staff Comfort working independently in a field-based role Valid driver's license, reliable transportation, and ability to travel within the service area Ability to pass required background checks; CPR/First Aid certification (or willingness to obtain) #J-18808-Ljbffr ECO Supportive Living
